Clone Stamped Spot Returns in O & A View?

I'm doing some work with images that were recorded on VHSC tape in 1995. Then it was digitised in 2007, then the image was screen shot from an MP4 video. There was a spot on the middle of the subjects nose, where from? who knows. Then added it to the show I'm producing, looked ok, then opened it in the O & A screen to add some text & the spot had come back. Anyone got a clue what is happening. Three images included, 1 the original image with the spot, 2. Clone stamped to remove spot. 3 Clone stamped image opened in O & A with spot returned.
